Diabetes is characterised by the increased blood sugar due to deficiency (or) diminished effectiveness of insulin. Lack of insulin affects the Metabolism of Carbo Hydrate, Protein, Fat, Water, and Electrolytes. The long standing metabolic derangements is frequently associated with permanent and irreversible functional and structural changes in the cells of the body particularly susceptible to vascular system. On the basis of Aetiology two main categories of diabetes are recognised, Namely Primary and Secondary Diabetes.
Primary Diabetes further classified in to
(1). Insulin Dependent Diabetes Mellitus (IDDM)
(2). Non Insulin Dependent Diabetes Mellitus (NIDDM).
IDDM (or) (type l ) most frequent in those less than 50 yrs old , and rapidly fatal ifnot taking adequate treatment. Genetic predisposition plays the vital role that damages the beta cells in the pancreas. Hyperglycemia occurs only when 90% of the beta cells had been destroyed.
NIDDM-( type 2 ) Environmental factors such as obesity, diet, inactivity,and pregnancy probably play a critical role, simple deficiency of insulin cannot entirely account for diabetic in NIDDM. Increased production of glucose from the liver and resistance to the action of insulin are characteristic of this NIDDM. Insulin resistance is also seen in obesity and the majority of middle aged diabetic patients are obese, most of the evidence shows that obesity is diabetogenic in those genetically predisposed to the disorder. Rising incidence of Diabetes in older people is related to increasing prevalence of obesity in the population as a whole. Non obese people also susceptible to NIDDM.
Secondary Diabetes
(A) Diseases affecting the pancreas such as carcinoma, pancreatitis
(B) Excessive productions of Growth hormone can causes permanent diabetes
(C) Adrenocortical hormones (or) cortico steroid therapy.
(D) Thyroid hormone in excess will aggrevate diabetes.
(E) Gestational diabetic : Hyperglycemia can occur during pregnancy temporarily.
(F) Liver diseases such as hepatitis and cirrhosis may associated with diabetes.
CLINICAL FEATURE OF DIABETES
Many patients are first noted to have glycosuria in the course of some routine examination. They may have had few (or) no symptoms.
